Diabetes Education Services
Presbyterian College School of Pharmacy Wellness Center offers a comprehensive program, Conversation Map®, designed to help participants manage diabetes through an understanding of proper nutrition, medications, monitoring, exercise, and reducing risks.
Active, small-group participation is the central component of Conversation Map® programs. During each session, participants and a trained diabetes educator engage in discussions about health information and how it relates to their personal experiences. After these conversations, participants are encouraged to create workable plans to use what they’ve learned to make long-term behavior changes that can improve their health.
This program is offered at no cost to PC employees or members of the surrounding community and neighboring counties. A referral from a primary care provider is preferred.
